The UNO Web Request form accommodates for different types of requests:
The first three types of request share many of the same items; the fourth is somewhat different from the others.
The first part of the request form allows the selection of the type of request as listed above. The second part for the first three types of requests have the following items:
Web Site Request (Departmental, Faculty/Staff, or Organizational)
(Persons requesting an Organizational Website please contact Student Involvement and Leadership at (504) 280-7083 to insure that the Organization is properly registered before requesting a site.)
Requestor-information--
Your name |
Name of the person requesting the web page. This person is probably (and should be) one of the "owners" or page masters of the page and will responsible for its maintenance |
UNO username |
UNO username of the requestor. The name of your LAN or e-mail account; i.e., the first part of your UNO e-mail address: the abced part of abcde@uno.edu . This cannot be an alias, like John.Doe or even John.Doe@uno.edu. This is needed for sending a verification of the request |
Department, Unit, Club, or Organization |
Name of the department, unit, club, or organization to which the requestor belongs and for which the web page will be created |
Phone number |
Phone number where the requestor can reached in case additional information is needed |
Chair/Director/Organization Advisor information--
Approver's Name |
Name of the department chair (or unit director); the person who actually approves the request for a web page for the department or for a faculty or staff member.
Note: In the case of a student organization or club, the Student Involvement &
Leadership (SIL) must approve the request. |
Approver's UNO username |
UNO username of the person who has to approve the request. This is the name of the person's LAN or e-mail account; i.e., the first part of the UNO e-mail address: the abced part of abcde@uno.edu . This cannot be an alias, like John.Doe or even John.Doe@uno.edu. This is needed for sending a the request to the UNO staff for creation
Note: In the case of a student organization or club, the Student Involvement &
Leadership (SIL) must approve the request and the username is sil. |
Preferred name |
A short name that will be visible only in the title bar of the display window |
Alternate name |
An alternate name for the title bar of the display window |

Why the web page is requested |
List of page masters |
List of LAN username of the individuals who will maintain the web page. Each username is put on a separate line. This is the name of the person's LAN or e-mail account; i.e., the first part of the UNO e-mail address: the abced part of abcde@uno.edu . This cannot be an alias, like John.Doe or even John.Doe@uno.edu |

Any additional comments or information that would be helpful to the staff member who will be creating the web page. (This item may be left blank.) |

The fourth web request involves adding more page masters to an existing or removing some page masters from the existing page.
