maintenance |

sending a verification of the request |

belongs |

additional information is needed |
| |

person who actually approves the request for a Listserv distribution list |
UNO E-mail address |
UNO e-mail address of the requestor's chair/director |
| |
List of owners |
Enter the UNO e-mail address of each owner of the list;
one address per line. Each of these addresses cannot be an alias;
i.e., the name must not contain a period. So you must use Jsmith@uno.edu
rather than John.Smith@uno.edu. |
Preferred name |
Name of the CPP distribution list. This represents the middle third of the complete name of the list, each third separated by a period.
The first third is the name of the college or department; the last third is
the word "List". Because the complete name will appear in the
Global Address List of Outlook, the first two thirds should be easily visible
in the narrow window. Therefore, these first two thirds of the complete name
should be no longer than 25 characters. While the name can contain any
mixture of upper and lower case letters, be aware that upper case letters are
"wider" than their lower case counterparts. For example, if the
College of Education requested a list with a name of "PR", then the
complete list name would be COED.PR.List. Hyphens "-" and
underscore "_" characters area allowed within the middle third of
the complete name. |
Alternate name |
Second choice for the CPP list to be created in case the
preferred name is already in use. |
Display name |
The full name that will be displayed in the
"To:" part of the heading of each sent message. |
Reason/purpose |
Brief statement why the CPP List is needed |
Extra comments |
Any comments the requestor may wish to communicate to the
UCC staff that will create the CPP List. (This item may be left blank.) |

Valid CPP combinations |
This drop-down list contains the available valid CPP
combinations. The requestor must select one or other combinations from this
list. More than one such combination may be selected (and saved) for
constructing the CPP distribution list. Once a valid CPP combination has
been selected, use the "Save this CPP" button to save this trio and
then select another. Each saved CPP trio is listed at the bottom of the
page with a check mark. Un-checking a specific trio will remove the
corresponding trio from the formal request for a CPP distribution list.
Therefore, a single CPP distribution list can have several component CPP
trios. |

Click this button to save the current CPP trio visible in
the first, left hand slot above; i.e., the "Valid CPP combination"
slot. This CPP trio will be listed at the bottom of the page as a reminder
of what CPP trios have been selected. Each saved CPP trio will have a
checkbox (which is automatically checked). Un-checking a specific CPP trio
will remove the corresponding trio when the lists of trios are submitted
later when the "Submit request for CPP list" button is clicked |

Submit request for CPP list |
Click this button to conclude the request for the CPP
distribution list which will contain the checked components as
indicated in the "List of selected CPP designations". If a
specific component is un-checked before this button is clicked, the
corresponding trio will be removed from the request to create the CPP List.
Clicking this button will send the request for a CPP
distribution list to the chair/director for approval. If approved, the
request is forwarded to UCC to create the list. A "Thank you" page
will be displayed.
If you want to submit a second request for a different CPP
list, you must use the Back button of the Browser until you reach the first
page of the CPP Distribution List Request Form, where you entered information
about yourself and the department chair or unit director. Once there, you
should make any appropriate changes and resume the process of indicating the
valid CPP trios for the second distribution list. |
