The following descriptions follow the same order as the items of the Listserv Request Form.
Name |
Name of the person requesting the Listserv. This person "owns" the List and is responsible for its proper use and maintenance. [Note that it is possible to have more than one owner.]

UNO username |
UNO username of the requestor. This is needed for sending a verification of the request. This username is the same as the LAN account or the first part of the UNO e-mail account; i.e., the abcde part of abcde@uno.edu.

Department/Unit |
Name of the department (or unit) to which the requestor belongs

Phone number |
Phone number where the requestor can reached in case additional information is needed |

Name |
Name of the department chair (or unit director); the person who actually approves the request for a Listserv distribution list

UNO username |
UNO username of the requestor's chair/director, the person who grants approval for the creation of the Listserv. This username is the same as the LAN account or the first part of the UNO e-mail account; i.e., the abcde part of abcde@uno.edu

List the owners |
Indicate the UNO username of each "owner" of the Listserv. An owner is empowered to initiate messages to the List (an editor is not). The username is the LAN account name or the UNO e-mail account; e.g. the abcde part of abcde@uno.edu. An alias (like John.Doe@uno.edu) is not acceptable.

Preferred name |
Name of the Listserv; a short name, perhaps containing hyphens "-" or under scores "_"; this name will have a suffix of "-L" to visually identify it as the name of a distribution list. For example, buzz-L

Alternate name |
Second choice for the Listserv to be created in case the preferred name is already in use. For example, uno-buzzwords-L
|
Display name |
Name to be used as the sender's name when messages are sent from the List. For example, Buzzwords Anonymous

Listserv type |
A private Listserv is one that requires potential members to request membership (from the owners)
|
Subscription type |
A Listserv can have open or closed subscriptions. An open subscription allows individuals to join (i.e., subscribe) or unsubscribe (i.e., conclude membership) as they desire. A closed subscription requires individuals to request membership in the List; similarly, a member must request a conclusion of membership. Note that the open option is atomically indicated, but can be changed.

Expiration date |
A date when the Listserv will no longer be needed; e.g., 1/31/2003 (because this represent a date six months after the conclusion of a conference and the Listserv was establish to accommodate communication for that conference)
If the Listserv will continue indefinitely, leave the entry with the word none

Who can post |
Which group of users can post (i.e., send messages) to the List: subscribers, editors, owners. ["Note that "subscribers" automatically includes the other two groups; "editors" includes "owners" but excludes "subscribers".] If "editors" is selected, list the UNO usernames of the editors in the next item; otherwise leave the following item blank. Note that the subscribers group is automatically indicated, but can be changed. The list of editor usernames are written one per line and may be copy-pasted from any text editor (like Notepad).

Want archives |
Archives are collections of all the messages sent to the members during a specific time period (e.g., monthly). Responding "Yes" allows for the periodic creation of archives, which may be reviewed by the members. Responding "No" means the Listserv will not have archives. Note that the Yes option is automatically indicated, but can be changed.

Want digests |
A digest is periodic collection of the messages that were sent to List member during a specific time period (e.g., monthly). One digest is sent instead of each individual message when each was posted. This option can be turned of or off by the individual List member

Extra comments |
Any additional comments the requestor may wish to communicate to the UCC staff that will create the Listserv. (This item may be left blank.)
